Church Communication Form

• To promote events approved through the Facility Use form
• Graphics are created based on details provided
• Promotions can be public or private, digital or physical
• Use online form to submit all requests

Check Requisition Form

• Request reimbursement for purchases related to church use
• Receipt must be attached when submitted
• Fill out pdf or word doc version
• Email form with payment requests to office@mapleridgechurch.org
