Parental Release Form

  • For all RidgeKids or RidgeYouth events
  • An adult must fill out this form
  • One form per child
  • Check each box to confirm your compliance
  • This form can be filled out and submitted online.

Event Form

Use this form when you need to:

• Add an event to the church calendar
• Reserve rooms or equipment (see Room Floor Plans)
• Request a specific room setup
• Promote an event (digital or print, public or internal)
• Make edits or updates to an existing event

WHERE TO FIND THIS FORM:
MRC App – fill out on phone
Go to More… tab, click on Forms icon, select Event Form.
Website – fill out on laptop
 Hover on Events in Menu and select Forms, select Event Form.

Communication Request

Use this form when you want to:

  • Share information with the church body that is not tied to an event
  • Typically used by Staff or Elder Board
  • Request communication such as:
    • Email
    • Push notification/Text message
    • Mailed letter/Postcard

Check Requisition Form

  • Request reimbursement for purchases related to church use
  • Receipt must be attached when submitted
  • Fill out pdf or word doc version
  • Email form with payment requests to office@mapleridgechurch.org
